This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services. The Case Manager is dedicated to facilitating clients' recovery and promoting their overall well-being and independence.Duties And ResponsibilitiesCoordinate and provide care athat is safe, timely, effective, efficient, and client-centered.Manage case assignments, draft service plans, and review case progress regularly.Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.Facilitate clients in achieving personal independence and assist in developing daily living skills.Perform other duties as assigned.Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.Supervision Given: This position does not supervise any staff.Supervision Received: The Behavioral Health Case Manager reports to their assigned Operations Managers.QualificationsStrong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.Knowledge of behavioral health practices and community resources.Ability to work independently and as part of a team.EducationHigh school diploma or equivalent is required; a college degree in Human Services is preferred.CertificationCPR certification is preferred.NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided).ExperienceMinimum of 3 years in behavioral health or a related field is preferred, but not mandatory.Physical EffortRequires sitting for extended periods and occasional lifting of up to 20 pounds.BackgroundMust pass all Federal and State background checks according to company policy.ScheduleFlexible scheduling options allow for accommodating personal availability, enabling as little or as much work as desired.BenefitsFlexible schedule.
