This role emphasizes proactive health maintenance, chronic disease management, and continuity of care within an innovative, value-driven model. With strong referral relationships, integrated care resources, and EPIC EHR, physicians enjoy clinical autonomy with robust support. This is a full-time, outpatient-only position with a predictable schedule, no hospital call, and a strong pathway to partnership.
