Bachelor's degree and experience working with children required Provide Therapeutic Training and Support (TT&S) and Therapeutic Mentoring (TM) services as part of the continuum of CBHI Services. TT&S services are provided as part of an In-Home Therapy team. Services are provided in homes or other community locations to youth and families enrolled in IHT through the Children’s Behavioral Health Initiative. The In-Home Therapy team provides strength-based therapy to youth under 21 years old who have social, emotional, and behavioral health needs and to the youths’ families. The TT&S staff works with the youth, the youth’s family, and the youth’s In-Home Therapist to assist the In-Home Therapist in implementing the therapeutic objectives included the youth’s treatment plan. Therapeutic Mentors provide one-to-one, strengths-based support to children, adolescents, and transition-age youth who have emotional/behavioral challenges. Therapeutic Mentors work in community settings alongside a clinical HUB that maintains a treatment relationship with the youth during the time the service is in place. Education and Licensure Required: o Bachelor's degree in a human service field from an accredited university o One year experience working with children/adolescents with serious emotional disturbance. o Will obtain Community Health Worker Certification through the state of Massachusetts within 2 years of hire in accordance with payer/regulatory requirements. Preferred: o Two+ years experience working with children/adolescents with serious emotional disturbance Experience Required: Experience working therapeutically with children and adolescents with behavioral health needs and their families. Preferred: Experience with strength-based assessment and development of treatment plans based on clear clinical formulations and knowledge of collateral systems. Demonstrated ability to work collaboratively within a team structure. Demonstrated creativity, cultural curiosity, and flexibility in responding to a variety of family, home, and community environments. Excellent written and verbal communication skills. Abilities · Required: o Must be 21 years of age. o Must have reliable transportation, proof of valid auto registration, auto insurance, and driver's license. o Ability to be flexible and responsive to client and family scheduling needs and crises between the hours of 8 am and 8 pm o Ability to meet deadlines and follow through with multiple projects simultaneously. o Ability to respond to and follow through with requests professionally. o Excellent verbal and written communication skills. o Must be able to sit at work station for prolonged periods of time, consistent with OSHA suggestions and requirements. o Must have the ability to access different community locations for meetings and sessions. o Must have ability to lift packages up to 10 pounds. · Preferred: o Bilingual candidates are strongly encouraged to apply (but not required). Essential functions: · Attend and/or travel to meetings/ appointments in various locations. · Maintain paper and electronic records in an orderly and timely manner. · If youth is enrolled in Intensive Care Coordination, participate in all care planning meetings and processes for the youth. · Ensure that all services are provided in a professional manner, ensuring privacy and safety. · Follow through with on-call policies and procedures. · Attends weekly individual supervision with a licensed clinician and biweekly team meetings. · Attends agency orientation and training on a yearly basis as required by the agency, program, and regulators/funders. · All other duties, as assigned[ND1] . As Therapeutic Training and Support (TT&S): 1. Assist the In-Home Therapist in implementing the therapeutic objectives of the treatment plan to address the youth's social, behavioral, and emotional needs. 2. Assist in developing and supporting an effective risk assessment and safety plan with youth and family. 3. Assist youth and caregivers in building various coping, life, social, behavior, and emotion management skills. 4. Provide education and practice in emotional identification, behavior management, and coping skills coaching. 5. Model positive interactions with youth and family to enhance problem-solving, limit-setting, safety planning, communication, and skill building to strengthen family relationships. 6. Attend monthly in-service trainings. 7. Assist in developing initial and ongoing teamwork with family members, formal providers, natural supports, and others who can support sustainable progress. As Therapeutic Mentor: 1. For each youth enrolled in services: a. Supports and coaches youth in developing age-appropriate behaviors, interpersonal communication skills, problem solving skills, and conflict resolution skills. b. Collaborates, as appropriate, with other providers and natural supports. c. Develops a treatment plan based off of HUB’s identified goals. d. Maintains weekly contact with HUB. e. Completes all necessary clinical documentation accurately and timely. f. Obtains authorizations. g. Attends treatment team/care plan meetings. h. Transports youth in the community. i. Ensures that services a provided to youth according to the performance specifications and CBHI guidelines.
