Physical Therapist

In this role, you will work closely with residents, families, and the care team to deliver thoughtful, evidence-based therapy that supports each resident's highest practicable level of function and well-being. This role reports to the Director of Rehabilitation.Full-Time and Part-Time opportunities availableWhat You'll DoEvaluate residents' physical function, mobility, and therapy needsDevelop individualized treatment plans based on clinical judgment, resident goals, and physician ordersProvide therapeutic interventions that improve function, safety, and quality of lifeMonitor progress, document outcomes, and adjust treatment plans as neededRecommend additional consultations or services when appropriateSupport discharge and transition planning, including restorative programs and follow-up recommendationsEducate residents, families, and caregivers on home programs, daily living skills, and adaptive equipmentSupervise Physical Therapist Assistants, students, and Rehabilitation Aides in accordance with applicable standards and guidelinesProvide in-service education and training for staff and caregivers as neededComplete documentation accurately, thoroughly, and on timeFollow all safety, infection control, and regulatory requirementsParticipate in fire, safety, and disaster preparedness drillsWhat Success Looks LikeHelp residents maintain or improve mobility and functionDeliver care that is resident-centered, respectful, and goal-orientedEnsure therapy services are aligned with each resident's care planHonor resident choices, including refusals of treatment, and document them appropriatelyMaintain a safe environment for residents, families, and team membersQualificationsGraduate of a CAPTE-accredited physical therapy program (or equivalent recognized program)Successful completion of required supervised clinical fieldworkPassed all required examinations for Physical TherapistsCurrent and unencumbered Physical Therapist license, registration, or certification as required by state lawWorking knowledge of physical therapy principles, practices, and documentation standardsCommitment to the APTA Code of EthicsOngoing dedication to professional growth through continuing education, networking, and professional developmentForeign-educated candidates must meet all state licensure requirements or qualify for an approved traineeship under the direct supervision of a licensed Physical TherapistPhysical RequirementsAdequate vision and hearing to perform essential job dutiesAbility to safely lift, transfer, and assist residents several times per dayAbility to stand and move throughout the facility for extended periodsAbility to push wheelchairs and beds, carry equipment, and reach overhead as neededAbility to perform tasks requiring reasoning, sequencing, judgment, and basic math skills
