We invite you to join us in our vision of "Making Lives Better!"Essential duties and responsibilities:Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care languageWraparound Care Coordinator will work with providers in the community, as well as family members across several life domains, to develop intervention strategies in a team-based approachNetworking with the community and family members in developing a team, working with very intensive needs of youth and their families, in guiding them through safety planning, crisis intervention, discovering strengths, developing underlying needs that could be causing the at-risk behavior, and empowering families to take leadership roles in their treatmentLearn the National Wraparound Institutes wraparound approach and is responsible to attend all required trainingsWork in a team approach with YES Waiver & Children's Behavioral Health Department working with youth ages 3-18 yearsOrganize culturally competent care activities and collaborating among all participants involved with the youth's careAdditional duties include: assessment & treatment planning, working within the team to ensure appropriate services are scheduled and delivered, engaging family by providing outreach and education, and taking a family & person-centered approach to help identify strategies and objectivesProvide services via phone & face-to-face, in both home and community-based settingsSpend at least half of his/her working time directly working with the youth, family, and/or family supportsAssist & collaborate with C&A BH Team in covering Amarillo and regional areas throughout the Texas PanhandleAdheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromisedRequired qualifications to be successful in the job:Bachelor's degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialingFor Case Manager II, minimum one (1) year of experience in behavioral health case management or social servicesFor Case Manager III, minimum two (2) years of experience in behavioral health case management or social servicesGraduate work in a related field may be substituted for up to one (1) year of experienceEmployees may be required to utilize their personal mobile phones for two-factor authentication (2FA).
