The Medical Social Worker – Home Health provides emotional support, counseling, and resource coordination to patients in their homes, ensuring holistic care that addresses psychosocial needs with dignity and respect.Assist patients and families with social, emotional, and financial concerns, teaching effective coping skills.Coordinate care plans, communicate clinical changes to the healthcare team, and facilitate education on social work interventions.Identify risks in the patient's environment and implement interventions to mitigate them.Perform discharge planning and ensure seamless communication among team members.Must haves:** MSW from an accredited program, current unencumbered Social Worker license, 1 year of healthcare social work experience, CPR certification, reliable transportation, and ability to commute within the area.Position details:** Part-time role with benefits including insurance, paid time off, holidays, 401K with match, and employee assistance programs.
