This role focuses on assessment, care coordination, and connecting clients with critical community resources to support recovery and stability.This position serves the FIVCO region and The Journey program, requiring collaboration across multiple community partners.Educational RequirementsBachelor’s degree in a behavioral science field required.Minimum QualificationsBachelor’s degree in a social or behavioral science fieldAt least one (1) year of experience providing case management servicesAbility to complete required training within six (6) months of hireKey ResponsibilitiesConduct client outreach, assessments, and individualized service planningProvide education on recovery, treatment options, and support servicesCoordinate care and make referrals to appropriate agencies and providersServe as the primary contact for referrals from local health departmentsCollaborate with community partners, including courts, DCBS, law enforcement, and sheltersMaintain accurate documentation and case recordsSupport clients in navigating complex systems and accessing servicesTraining & DevelopmentDBH-approved Targeted Case Management training provided within six (6) months of hireOngoing continuing education required to maintain certificationIdeal CandidateExperience working with high-risk or underserved populationsStrong organizational and communication skillsAbility to manage a caseload independently while collaborating with a teamPassion for supporting maternal health and recovery servicesThis is a full time position
