Targeted Case Manager

This policy includes, but is not limited to: recruitment, employment, promotion, demotion, transfer, termination, compensation, benefits, layoffs, training, and social and recreational programs.Salary: $35,000 - $39,375 annuallyEducational RequirementsBachelor’s Degree in Human Services or a related field requiredMinimum of one year of post-graduate experience requiredDuties & ResponsibilitiesProvide case management services to assigned consumers in accordance with individualized treatment and service plansCoordinate and monitor services to support consumers’ behavioral health, medical, social, and community resource needsMaintain accurate, timely, and current documentation in compliance with agency, state, and federal requirementsCollaborate with consumers, families, providers, and community partners to ensure continuity of careAssist consumers with accessing community resources, benefits, and supportive servicesParticipate in treatment team meetings and contribute to care planning effortsAdhere to all Pathways policies, procedures, and confidentiality standardsPerform additional duties as assigned within the scope of the positionSkills & KnowledgeExperience working in a behavioral health setting preferredStrong communication, organization, and interpersonal skillsAbility to manage multiple tasks and maintain accurate documentationCompetency in using computers, electronic health records, and other electronic devices required for job dutiesAbility to work independently while collaborating effectively within a multidisciplinary teamThis is a full time position
