Apply now and we'll connect you with a recruiter who'll reach out with more details.Minimum RequirementsBachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree from an accredited PT program.Current, active Arizona Physical Therapist license required prior to start of assignment.Proof of current CPR certification required.Current valid driver's license, reliable transportation, and insurance required due to home health travel requirements.1-2 years experience as a Physical Therapist in acute care or rehabilitation setting required.Previous home health experience preferred.Ability to perform patient assessments, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams as part of standard practice.Ability to safely instruct and assist patients with transfers and durable medical equipment use, and to order/adapt equipment for home environments.Ability to perform physical tasks including lifting, bending, pulling, pushing, prolonged standing and walking.Ability to work a flexible schedule and travel locally as required by the role.
