Referral Coordination Team Lead

A company is looking for a Referral Coordination Team Lead to oversee referral processes and enhance partnerships in a remote setting. Key Responsibilities Oversee the intake, processing, and tracking of incoming referrals Manage outreach efforts related to referrals and monitor engagement documentation Collaborate with Community Partnerships Managers to support external referral relationships Required Qualifications 3-5+ years in healthcare operations, referral management, care coordination, or managed care 1-2+ years of management experience Experience with Medi-Cal, ECM, CalAIM, or community-based programs strongly preferred Strong process design and data analysis skills Experience with Google Workplace and/or Microsoft Office, specifically Google Sheets or Excel
