You will focus on preventing, diagnosing, treating, coordination of care, preventive care and health maintenance of their patients with a wide range of illnesses and chronic conditions. They must be cognizant of, and comply with, all company policies and procedures, as well as pertinent state and federal regulations. Provides quality, comprehensive Palliative health care services to the patients' homes.Essential Job FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.The Nurse Practitioner provides comprehensive palliative and advanced care services which fall under their field of training, in conjunction with the Palliative Care Medical Director. Carry out the assessment, management and monitoring of patients in the Palliative Care Program, providing care, education, training, and support, to alleviate suffering and improve the quality of life of patients and their families. They must acknowledge and comply with the state Nurse Practice Act and all policies and procedures, as well as pertinent state and federal regulations. Evaluates patients to verify eligibility for palliative care programs.Assesses and manages the most complex patients, as well as those in the end-of-life state, by performing a thorough clinical assessment and designing an individualized care plan.Take appropriate action to maintain a safe environment. Check equipment and supplies prior to use for function, inspection and expiration date; reports malfunction of medical equipment and safety issues to supervisor. Check medical records and follow up on missing results prior to the patient's appointmentEducates and provides assistance to patient and caregiver(s) in understanding and completing advanced directives based on the patient’s goals and wishes. Provides instructions and solves doubts and concerns to patient relatives/caregivers on managing symptoms and general care indications.Refers patients to in-House or external specialists/services when clinically indicated.Timely refers patients to hospice when requiredParticipates in the care of ACP program members, through multidimensional evaluation and care plans that include patients and families counseling.Obtains health history and performs physical examinations to make correct diagnoses.Prescribes medications in accordance with state statutes and professional practice guidelines.Maintains complete and accurate health records for each patient in accordance with policies and procedures.Ensures continuity of care by providing comprehensive and integrated services in a timely manner through all settings of care.Provides instructions and solves concerns to patient relatives/caregivers on managing symptoms and general care indications. Educates patients and caregivers on topics such as treatment, medications and side effects, illness progression, diet and nutrition, nursing care, medical adherence, and crisis anticipation and prevention.Implement interventions to manage patients with recurrent admissions. Assess and monitor patients with poor symptom control to improve patient wellness and avoid unnecessary admissionsActively participates in meetings and committees preparing the pertinent information for the adequate development of these spaces and communicating challenges and improvement plans.Identify and report to leadership immediately any concerns or issues that places the team, patient, or caregiver at risk.Actively participates in the development of research activities and scientific events, seeks symposiums and congresses on palliative care which the staff can benefit from.Identifies and reports to leadership immediately any concerns or issues that places the team, patient or caregiver at risk.Perform duties autonomously by proper task scheduling to meet workload expectations.Fulfills with other assignments related to the role according to background and capabilitiesAssumes responsibility for personal growth.
