Our interdisciplinary team focuses on restoring independence, improving safety, and enhancing quality of life for patients with functional limitations.We are seeking a licensed Occupational Therapist who thrives in autonomous practice, values clinical excellence, and understands the unique impact of therapy in the home setting.Position SummaryThe Occupational Therapist provides skilled occupational therapy services to homebound patients in accordance with the physicians plan of care, applicable state and federal regulations, and agency policies. Services focus on enabling patients to safely perform activities of daily living (ADLs), instrumental activities of daily living (IADLs), and functional mobility within their home environment.Essential Duties & ResponsibilitiesThe Occupational Therapist will:Conduct comprehensive occupational therapy evaluations in the patients homeDevelop, implement, and revise individualized Plans of Care (POC) in collaboration with the physicianProvide skilled interventions including: ADL and IADL retraining (bathing, dressing, toileting, feeding, home management)Upper extremity strengthening and coordinationFine motor and cognitive-perceptual trainingEnergy conservation and work simplification techniquesAdaptive equipment assessment and trainingHome safety and environmental modification recommendationsAssess patient progress and modify treatment plans based on clinical responseEducate patients and caregivers on: Safe performance of daily tasksUse of adaptive equipment and assistive devicesStrategies to maximize independence and prevent injuryComplete accurate, timely, and compliant documentation in accordance with: Medicare Conditions of ParticipationTexas HHSC home health regulationsAgency policies and proceduresParticipate in interdisciplinary communication and case conferencesCoordinate care with physicians, nursing staff, and therapy team membersSupervise Certified Occupational Therapy Assistants (COTAs), if applicable, per Texas regulationsFollow infection control, safety, and HIPAA requirementsParticipate in discharge planning when therapy goals are achieved or services are no longer medically necessaryQualificationsRequired:Current Texas Occupational Therapist license in good standingGraduate of an accredited Occupational Therapy programMinimum 1 year of experience preferred (home health experience strongly preferred)Knowledge of Medicare home health documentation and regulatory standardsValid Texas drivers license, reliable transportation, and auto insuranceCurrent CPR/BLS certificationAbility to work independently and manage a mobile caseloadPreferred:Experience with OASIS documentationFamiliarity with home health EMR systemsStrong patient and caregiver education skillsPhysical & Mental RequirementsAbility to travel to patient homes within assigned service areaAbility to assist patients with transfers and functional tasks as neededAbility to lift, push, pull, bend, reach, and perform therapeutic activitiesAbility to exercise sound clinical judgment in an independent environmentWhat We OfferCompetitive per-visit or salaried compensation (based on role and experience)Flexible scheduling and manageable caseloadsSupportive leadership with clinical insightOpportunity to work with a growing, mission-driven Texas home health agencyProfessional autonomy with strong administrative supportEqual Opportunity EmployerApplesoft Homecare Services LLC is an equal opportunity employer and complies with all applicable federal and Texas employment laws.
