Travel Physical Therapist | Fremont, California (Hiring Immediately)

A dynamic opportunity is available for a skilled Physical Therapist to deliver therapy services in a hospital and home health environment. This contract role offers a chance to serve a diverse patient population while maintaining a flexible approach to caseload and area assignments. Responsibilities include conducting evaluations, implementing patient-centered treatment plans, and supporting patients across designated territories-either in the Richmond to San Pablo corridor and Oakland region or the Vallejo to Napa area. Additional travel and weekend work may be needed to fulfill weekly visit expectations and support patient needs.Qualifications: Active California Physical Therapy license required Current BLS certification Valid driver's license, vehicle registration, and auto insurance Up-to-date immunizations: TB (annual), MMR, Varicella, HepB, Tdap (within 10 years), Flu (annual) Ability to clear a drug screen and physical exam Previous experience in hospital or home health therapy preferred Willingness to travel across multiple counties as needed Commitment to providing care to Covid+ and Covid PUI patients when required Schedule/Hours: 36-hour guarantee applies based on productivity requirements (minimum of 27 points per week) Flexibility to work weekends as needed Occasional travel outside primary area to maintain a full caseload Key Responsibilities: Evaluate and treat patients consistent with PT best practices, including OASIS and recertification visits Deliver therapy to a range of patients, completing all documentation, follow-up calls, and care coordination Adapt to variable scheduling and territory assignments to meet patient needs Maintain compliance with all credentialing, documentation, and regulatory standards This role is ideal for candidates who thrive in an autonomous, mobile work environment and value patient-centered care in the therapy field.
