The role focuses on accurate assessment, diagnosis, treatment, health counseling, chronic condition management, and disposition planning to achieve positive patient outcomes. This position reports to the Practice Manager.Key ResponsibilitiesProvide primary and family evidence-based care, including assessment, diagnosis, treatment, and management of acute and chronic health conditionsDeliver health counseling, preventive care, and disposition planningConduct routine examinations and diagnostic testing to support patient care decisionsEncourage patient engagement in healthcare through health screenings, behavioral health and wellness services, and chronic condition managementCollaborate frequently with a clinical care team and other healthcare professionals to ensure comprehensive plans of carePerform administrative duties including opening/closing the clinic, inventory management, patient follow-up, verifying insurance, collecting payments, and maintaining a clean clinical environmentManage patient appointments and documentation using electronic systemsOperate independently while maintaining high standards of quality, safety, and complianceQualifications & RequirementsMaster's Degree from an accredited Family Nurse Practitioner programCurrent National Board Certification as a Nurse PractitionerActive State of Missouri license to practice in the Advanced Practice Nurse roleMinimum of 1 year of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care preferred; otherwise, successful completion of a primary care training programVerifiable High School Diploma or GEDCandidate questionnaire must be included on the resumeWorking knowledge of quality measurement and chronic disease managementStrong verbal, written, and electronic communication skillsExcellent organizational skills with the ability to multitaskSelf-motivated, adaptable, and able to solve problems independentlyAbility to practice autonomously without direct supervisionProficiency with information management systems and healthcare technologyWillingness to obtain multi-state licensureCapacity to collaborate regularly with professional colleagues to ensure quality patient careAdditional InformationAll positions are designated as safety sensitiveDEA registration is not required for MissouriBenefits are available to full-time employees after 90 days of employment; 401(k) eligibility with company match after 1 year of serviceINDKS
