This position is responsible for conducting Health-Related Social Needs (HRSN) screenings, eligibility assessments, intake coordination, and service referrals for individuals seeking support with housing, food insecurity, transportation, care management, and other community-based services.The Intake & Screening Specialist plays a critical role in ensuring members are connected to the appropriate level of support quickly and efficiently while maintaining accurate and compliant documentation required for Medicaid, SCN, and HRSN service delivery. This role works closely with Care Managers, Housing Specialists, and administrative staff to ensure smooth onboarding and continuity of care.ResponsibilitiesConduct intake interviews, screenings, and eligibility assessmentsReview and collect required member documentationCoordinate onboarding into care management and support servicesMaintain accurate and timely documentation in agency systemsRespond to incoming referrals and inquiriesSchedule follow-up appointments and service coordinationCommunicate with members, providers, hospitals, and community partnersEnsure compliance with agency, Medicaid, and HRSN documentation requirementsAssist with referral tracking and intake reportingSupport member engagement and connection to community resourcesQualificationsStrong communication and organizational skillsExperience in behavioral health, social services, healthcare, or community outreach preferredAbility to manage multiple tasks and maintain accurate documentationComfortable working with vulnerable populations and individuals experiencing social care barriersBasic computer and documentation skills requiredKnowledge of Medicaid, care coordination, or HRSN services preferredEmployment DetailsFull-Time PositionHourly, Non-ExemptStarting Pay: $20/hourOpportunities for growth within the organizationThe Pay Range For This Role Is20 - 22 USD per hour(Remote)
