WORK Inc. is seeking a Residential LPN to join its nursing team, assisting the Director of Healthcare Services in providing clinical nursing supports for all medical services for individuals attending the adult residential programs.
The ideal candidate will provide nursing support to residential staff in 4/5 group homes with a high degree of autonomy and be able to independently identify individuals needs and provide appropriate intervention. Supervision and support from Director of Health Services.
Hours:
The position is 30hrs, per week with flexible hours and no on call responsibility. These are well established programs with excellent management retention and care. This position is based in the Residentialgroup homes and includes going to medical visits,hospitals and rehabilitation centers etc. when necessary.

Responsibilities Include:
Nurse case management, collateral communication with caregivers, conducting MAP audits, medication administration support, staff training and support, discharge planning, nursing assessments.
Qualifications
Current MA license as an LPN
MAP Certification or ability to become certified.
Must have and retain an active drivers license
