Physical Therapist - Home Health, Part Time

Touchstone Health, a full-service home health agency, is seeking a qualified and compassionate Physical Therapist to care for our patients in the Bandera and Hondo areas. Under the supervision of the Clinical Manger and in accordance with the interdisciplinary plan of care, our therapists evaluate, establish, and implement individual programs of care for our patients to increase functional ability and reestablish independence at home.Duties & ResponsibilitiesPerforming home visits to evaluate patient's level of functionDeveloping an individual plan of care based on the client's personal goalsImplementing the treatment program through direct treatment of the patientReassessing clinical signs and symptoms to determine effectiveness of programInstructing patient, family, and staff regarding participation in the treatment programCollaborating with PTA and additional interdisciplinary staff member to improve treatment programRequirementsQualifications & Requirements:Degree in physical therapy from a university accredited by the American Physical Therapy AssociationLicense to Practice by the TexasComprehensive knowledge of the principles, concepts, and methodology required of an physical therapist and its practice application in the treatment of home health patients at homeExcellent Communication and documentation skillsWillingness to work with a interdisciplinary teamValid Texas driver’s license and reliable, insured transportationBenefitsBenefits & Allowances:Positive work environment based on collaborative teamworkExceptional patient outcomes based on complete knowledge of home environmentDirect patient care without time restrictions to build patient relationshipsFlexible work scheduleCompetitive Compensation401(k)
