We’re looking for dedicated professionals ready to support, advocate, and empower families as they work toward long-term stability and permanency.Position Overview: Permanency Case ManagerIn this impactful role, you will work directly with children in the Managing Conservatorship of DFPS and their families. You’ll assess needs, develop plans, and coordinate services to help families safely reunify—or ensure every child achieves permanency in the most nurturing setting possible.Key ResponsibilitiesAs a Permanency Case Manager, you will:Manage a caseload and guide families throughout the permanency processAssess child and family needs, safety concerns, and strengthsFacilitate permanency planning and ensure safe, stable placementsConduct ongoing searches for potential Kinship placementsVisit children and families monthly—or more often as neededProvide post-reunification support until court dismissalPartner with public and private agencies to coordinate servicesPrepare assessments, court reports, and provide testimonyMaintain complete and accurate case documentationPrepare families for permanency through training and home visitsCollaborate with Adoption Coordinators to prepare children for adoptionParticipate in 24/7 on-call rotation and respond to crisesWork with Care Management and PAL staff to support placements and older youthCompile and submit monthly reports and dataChampion cultural diversity and sensitivity in all interactionsUse personal or agency vehicles to transport children safelyQualificationsBachelor’s Degree requiredWhat We OfferTFI Family Services provides a comprehensive and supportive benefits package, including:Competitive salaryHealth, dental, and vision benefits401(k) with company matchPaid personal leave & holidaysLongevity bonusAnd more!We believe in investing in the people who make our mission possible.Ready to make an impact?
