Job Title: Licensed Practical Nurse (LPN) – OPWDD Residential Program Location: Brooklyn, NY Schedule: Monday to Friday 9a-5p Salary: $70,000 to $71,253/yearMake a Difference Every Day Seeking a dedicated Licensed Practical Nurse (LPN) to support individuals with developmental disabilities living in our OPWDD residential settings in Brooklyn. This role is ideal for a compassionate nurse who enjoys hands-on care, building relationships with residents, and working as part of a collaborative interdisciplinary team.Key ResponsibilitiesProvide nursing care to individuals residing in OPWDD residential homesPerform phlebotomy/blood draws and coordinate lab work as neededAdminister medications and treatments in accordance with physician orders and agency protocolsMonitor residents' health status and report changes to the RN or medical teamMaintain accurate documentation and ensure compliance with OPWDD and regulatory standardsCollaborate with residential staff to support each resident's individualized health care planProvide health education and guidance to direct support staffQualificationsActive NYS Licensed Practical Nurse (LPN) licenseProven blood draw / phlebotomy experience (required)Experience working in OPWDD, developmental disabilities, or residential settings strongly preferredStrong communication and documentation skillsCompassionate, reliable, and team-oriented
