As part of our continued growth andcommitment to expanding access to care, we are launching telemedicine servicesthroughout New York to deliver high-quality gynecologic care to patients statewide. Weare seeking W2 Remote Patient Care Coordinators to support both our in-personlocations as well as our telemedicine expansion.Position SummaryThe Patient Care Coordinator is a licensed registered nurse (RN) responsible forcoordinating comprehensive, patient-centered care across both brick-and-mortar andtelemedicine platforms. This role serves as a central point of coordination betweenpatients, providers, and care teams, helping deliver a seamless, high-quality careexperience.Key ResponsibilitiesServe as a clinical liaison for patients.Perform clinical intake, triage, and ongoing patient assessments within scope ofpractice.Collaborate with physicians, advanced practice providers, and interdisciplinaryteams to develop and implement care plans.Provide patient education regarding conditions, treatments, medications, andfollow-up care.Track and manage test results, referrals, prescriptions, and care transitions.Identify and escalate urgent or complex clinical issues appropriately.Support informed consent processes and ensure compliance with regulatory andorganizational standards.Assist patients in navigating care pathways, including specialty referrals andexternal services when needed.Ensure accurate, timely, and compliant documentation in the electronic medicalrecord (EMR).Monitor care gaps and proactively engage patients to support adherence andoutcomes.Participate in quality improvement initiatives and cross-functional workflowoptimization.QualificationsActive, unrestricted Registered Nurse (RN) license in NY (multi-state licensurepreferred).Minimum 2–3 years of clinical experience in ambulatory care, women's health,telemedicine, or care coordination.Strong clinical triage and assessment skills across diverse patient populations.Experience working in or supporting multiple specialties preferred.Proficiency with EMR systems.Excellent communication, organizational, and patient engagement skills.Ability to work independently and manage a high-volume, fast-paced virtualenvironment.Meticulous attention to detail.Preferred QualificationsExperience in multispecialty or integrated care settings.Background in care coordination, case management, or population health.Familiarity with telemedicine workflows, regulatory standards, and documentationrequirements.Core CompetenciesClinical judgment and prioritizationCross-specialty care coordinationPatient-centered communicationAdaptability in a virtual environmentCollaboration and accountabilityWork Environment & ScheduleFully remote position.Flexible scheduling; may include evenings, weekends, or rotating shifts.Must maintain a private, HIPAA-compliant workspace and reliable high-speedinternet.Why Join UsInnovative, multispecialty virtual care modelCollaborative, interdisciplinary team environmentOpportunity to impact patient care at scaleFlexible remote work structure
