Job Title: Patient Navigator Non ClinicalLocation: Trinitas Regional Medical CenterDepartment Name: NJ SNAP Navigator GrantReq #: 0000233275Status: HourlyShift: DayPay Range: $21.69 - $30.64 per hourPay Transparency:The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey.The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.Job Overview:The RWJBH Social Impact and Community Investment SICI SNAP Navigator will assist patients and community members with screening, providing eligibility information and enrollment regarding SNAP Supplemental Nutrition Assistance Program benefits.The Navigator will provide services in RWJBH clinical spaces, outreach events, and with community partner organizations in the county/counties in which they are assigned.This role will operate in alignment with RWJBH s commitment to combatting food insecurity in the communities we serve. We are looking for an enthusiastic team member who is passionate about making healthy food available for our neighbors in need. The SNAP Navigator will report to the SNAP Navigator Program Manager who reports to the SVP, Social Impact and Community Investment.Qualifications:Required:High School Diploma or equivalentComprehension of food security/food access issuesIndividuals with experience in Public and/or Community Health are best suited for this job, however relevant work experience in benefits enrollment programs will also be consideredValid drivers license and vehicle, automobile/vehicle with valid insuranceThis position requires excellent organizational and interpersonal skillsSolution-driven, self-motivated, empatheticProficiency in Microsoft Office SuitePreferred:* Bilingual Spanish or Haitian Creole speaking preferredEssential Functions:Receive referrals from clinical and non-clinical staff, community partners and through direct inquiries to screen and enroll individuals in SNAP benefitsAssist in completion of new and renewal benefits applications on behalf of patients/community membersProvide follow up assistance regarding status updates on completed applicationsCoordinate scheduling with community partner organizations to provide servicesMaintain and foster new relationships with partner organizationsParticipate in recurring state and partner organization led trainings to reach and maintain expertise in program eligibility requirements and application proceduresCollect enrollment and additional data for monthly departmental and quarterly state reportingFlexibility to work event-based scheduling including occasional evenings and weekendsOther Duties:Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
