The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.ResponsibilitiesCoordinate care for heart transplant patients, including immunosuppression monitoring and prophylactic antibiotic management. Monitor for organ rejection, infection, and manage routine patient monitoring duties. Assess graft function and general health status, determining need for additional assessment or intervention. Coordinate changes to medication regimens with medical teams and oversee laboratory results for necessary actions. Communicate with multidisciplinary team members, patients, and families regarding patient care issues. Facilitate patient activities with consulting departments and assist in evaluating care of the transplant patient.QualificationsValid CA RN License with AHA BLS certification required. Minimum 1 year of ambulatory care experience. Experience in cardiac nursing, specifically in heart failure and heart transplant management. Ability to float to other UCSF facilities as needed. Experience with Epic charting system is essential.
