Observes and reports patients' vital signs, symptoms and general condition to the appropriate individual(s). Records on patient charts observation of patient's responses to nursing care rendered.Maintains environmental conditions conducive to the comfort and safety of patients. Assists with unit management and/or assume charge of unit as assigned.Cobble Hill Health Center is a not-for-profit long-and short-term care facility providing services to the disabled and elderly for over 40 years.RequirementsActive LPN license in the State of New York.CPR certificationHave the ability to observe and identify the resident's needs, report to the RN/Supervisor and collaborate with the RN in making clinical decisionsPrioritize and coordinate activities of the nursing unitBe an excellent problem solver and have strong decision-making skillsHave demonstrated leadership abilitiesHave prior experience working in a skilled nursing facility (preferred, but not required)Experience with SigmaCare (preferred, but not required)If you are ready to make a difference in your life and the lives of your colleagues and patients, apply today!
