If you’re excited to shape the future of home-based care, here’s what the role involves.POSITION OVERVIEW TheLPNprovides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient.ESSENTIAL DUTIES AND RESPONSIBILITIESProvides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of CareProvides Skilled Nursing visits according to visit schedule and notifies agency of need to alter scheduleReports significant findings to patient's physician and Clinical Supervisor as they occurSubmits completed skilled nursing visit notes per agency policy on designated days as requested by Clinical SupervisorSubmits change orders per agency policyProvides assistance with activities of daily living (ADL's) which may include personal hygiene, changing of bed linens, dressing and ambulation and transfers as the patient need arises and/or as instructedMaintains open lines of communications to all members of the continuum of care teamAttends staff meetings, team conferences and educational in-services per agency requirementsIs responsible for obtaining information provided at skilled nurse in-services and meetings and demonstrates appropriate follow-up related to information given at meetings and in-servicesParticipates in Process Improvement (PI) program by assisting with collection of data and serves on PI team upon requestFollows agency policies and proceduresPerforms these and all other duties as assigned by the AdministratorMaintains productivity level at 90%Participates in discharge planning process per Medicare Criteria and agency policy and procedureResponsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly CareOther duties as assignedREQUIRED KNOWLEDGE, SKILLS AND EXPERIENCEA current unencumbered State Professional Nurse LicenseActive CPR CertificationMust maintain a valid driver's license and good driving recordAn automobile to be used for workThe ability to make sound professional clinical judgmentThe ability to assess and document patient needs and meet those needsProficiency in clinical skillsExcellent verbal and written communication skillsProficiency in personal computer use including e-mail, clinical, word processing, and spreadsheet softwareAble to lift 40 pounds from floor to shoulderRepetitive walking, standing, sitting, bending, and use of handsAble to drive car 2-4 hours per dayPREFERRED KNOWLEDGE, SKILLS AND EXPERIENCEOne year of experience as a professional nursePerks and Benefits (What we offer):Competitive salary and multiple health benefit optionsCareer growth and developmentMentorship and continued learning opportunitiesEngaging and mission driven workplace401(k) matchingMileage reimbursement (if eligible)Every person on our team helps shape the future of Alpine Care Home Health.#J-18808-Ljbffr
