Our services are covered by Medicare, Medicaid, and most third-party payers, ensuring patients and families never receive a bill for care.Old Colony Hospice and Palliative Care is hiring for full-time and part-time Home Hospice Aide/CNA positions.Position 1: Field CNAPosition 2: Hospice Home HHA or CNA (3p- 11p - multiple days/weekends, inquire for more details). The Hospice Aide (CNA) is responsible for providing personal care and designated homemaking services for assigned patients and families whether delivering hospice care in the patient’s home or in a skilled nursing facility during a regularly scheduled shift, as well as provide weekend and holiday coverage as may be required or requested. Essential duties include, but are not limited to:Provide direct patient care (whether in the home or other location) consistent with the written instructions and/or as assigned including: bathing, oral hygiene, nail/skin/hair and foot care, pressure relief, dressing and feeding, transferring positions or locations; report to the charge nurse upon arrival and before leaving a skilled nursing facilityAssist patient with prescribed exercises, with ambulation and repositioning as may be part of the plan of care or as directedProvide light housekeeping duties such as: changing the bed, doing patient’s personal laundry, tidying the patient’s room, doing patient’s dishes, and keeping the kitchen and bathroom neat and clean after usePrepare and serve meals to the patient and/or prepare food to be served laterMaintain appropriate communication with the Hospice Aide Scheduling Supervisor and/or Clinical Manager on a daily basis including checking telephone messages, text or e-mail every morning before leaving to visit the first patient and between each patient visit.Maintain accurate records/documentation of patient care given in the electronic record system application built into the organization-supplied smartphoneThis job requires travel to and between patients homes and other locations where we visit patientsOld Colony Hospice is an Equal Opportunity EmployerPlease visit: www.oldcolonyhospice.org/careers
