'Responsibilities' 'Provide personal care and assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming. Monitor and record patient observations including vital signs. Assist with medication administration and medical documentation. Support clients with Alzheimer's, dementia, and memory care needs. Use EMR/EHR systems accurately for documentation. Ensure patient safety through diligent observation and adherence to protocols. Collaborate with healthcare professionals to deliver comprehensive home health services. Maintain a clean and safe environment during visits.' 'Qualifications' 'Valid certification as a Home Health Aide or Certified Nursing Assistant. Prior experience in home health or assisted living preferred. Knowledge of medical terminology and HIPAA regulations. CPR certification required. Experience caring for seniors or individuals with disabilities is highly valued. Compassionate attitude with excellent communication skills. Ability to work independently and as part of a team. Reliable transportation and flexibility in scheduling.'
