We're committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.Physical Therapist (PT):Full-Time Hours: Monday - Friday (40 Hours/Week); Flexible ScheduleAnnual Salary: $80,000 $110,000 (Based on a Per Visit Rate)Coverage Location: Logan, OH & Surrounding Areas (Including Lancaster, OH and Laurelville, OH)Travel: Required + Milage Reimbursement ($0.48/Mile)Responsibilities:Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.Assess and evaluate therapeutic, rehabilitative/functional status to develop, implement and direct physical therapy programs that focus on rehabilitation.Evaluate the patient's home environment, assess patients for muscle strength, mobility, gait, ROM potential for rehab.Direct patients and families/caregivers in the use and care of therapeutic appliances.Report the patient's reaction to treatment or changes in their condition to their physician.Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.Identify patient and family/caregiver needs for other home health services and refer as necessary.Prepare and submit clinical and progress summaries based on the attainment of goals.Participate in discharge planning for the patient.Provide in-service education programs for nursing organization personnel as needed.Participate in the peer consultation process.Supervises PTAs according to organization policy and state regulations.When therapy is the only skilled service, instruct, supervise and evaluate home health aide care every two (2) weeks.Collaborate with other team personnel to achieve well-rounded care.QualificationsRequirements:Active physical therapy license in the state of Ohio.Bachelors, master's or doctorate in physical therapy from an Accredited Program, Required.Possess and maintain a current CPR certification.Recent TB Test.Valid driver's license and reliable transportation with active automobile insurance.Qualifications:Previous experience in physical therapy, rehabilitation, or other areas preferred.Two years of appropriate experience as a physical therapist preferred.Community/home health experience is preferred.Demonstrates good verbal and written communication, and organizational skills.Ability to handle physical workload.Ability to build rapport with patients.Strong leadership qualities.BenefitsHealth InsuranceDental InsuranceVision Insurance401(k)401(k) MatchingFlexible Spending AccountPaid Time OffPaid Sick TimeMileage Reimbursement ($0.48/Mile)Referral ProgramFlexible Schedule
