Physical Therapist

PT Home HealthSpringfield, ILAs a Physical Therapist on our client's Home Health team, you'll deliver high-quality physical therapy services to patients in their homes. This role involves evaluating functional abilities, developing individualized treatment plans, and helping patients improve independence as well as physical and cognitive functioning. You'll also provide education and guidance to patients and caregivers, oversee appropriate supervision, and document patient progress throughout the course of care.
