East Haven Nursing and Rehabilitation Center - Seeking an experienced Licensed Dietitian for a 200-bed facility. Responsabilities include :Provides nutritional care for residents in accordance with the facility policies and procedures and as consistent with guidelines developed by the New York State Department of Health, and Federal governing bodies of long-term care facilitiesAssesses nutrition needs and identifies nutrient requirements according to specific needs of residentsAccurately and appropriately determines the nutritional status of residents through completion of nutritional assessmentsAssesses the adequacy of calorie, macro-nutrient, and fluid intakeCollaborates with the resident and/or family member, and health care team to assess nutrition and nutrition education needs of patientsIdentifies need for nutrition supplementation for residentsContinuously monitors and evaluates the nutrition care and outcomes of residents
