Client Referral & Outreach Coordinator (IDD Services)

JOB DESCRIPTION The Community Outreach Coordinator – Client is responsible for building and maintaining referral relationships that lead to new client intakes for IDD services. This role focuses on community relationships, referrals, and follow-up, ensuring a steady and appropriate flow of clients aligned with caregiver capacity. This role does not provide direct services and does not manage cases. Referral and Client Development: Build relationships with brokerages, CDDPs, support coordinators, and community partners. Conduct outreach to referral sources and community organizations. Educate partners about agency services and capacity. Follow up on referrals and inquiries promptly. Schedule meetings, or intake conversations. Coordinate handoff to Client Intake and Case Management teams. Track referral sources and intake outcomes. 2. Community Outreach and Events: Attend community events, provider meetings, and networking opportunities. Build referral relationships with community organizations, schools, doctors’ offices, etc. Maintain regular contact with referral partners. Represent the agency professionally and consistently in the community. 3. Collaboration: Collaborate closely with Client Intake, Case Managers, Area Directors and Operations. Communicate client demand trends and capacity needs to the Director of Operations. Align client intake flow with caregiver availability. 4. Performance Metrics: Qualified client referrals per month. Conversion rate from referral to intake. Referral source engagement. Timeliness of follow-up. Alignment of client demand with staffing capacity. 5. Community Events/Involvement: Actively engage in community involvement initiatives as needed. Assist in planning community events as needed. Attend company events within proximity as needed. Volunteer – minimum of 4 hours, maximum of 8 hours of mandatory paid volunteer hours per year. 6. General Responsibilities: Create quarterly newsletter content for area. Stay informed on ODDS updates, policies, procedures, and transmittals. Reviews task schedules and submit for approval as needed. Responsible for being on call one weekend a month. General office management tasks as required. EDUCATIONAL REQUIREMENTS/LICENSES/ CERTIFICATIONS High school diploma, GED, or equivalent experience 1+ year experience in client outreach, intake coordination, or community engagement. Strong relationship-building and communication skills. Organized and detail-oriented. Comfortable with follow-up and outreach. Ability to manage multiple potential clients at once. Reliable transportation for community meetings and events. PREFERRED REQUIREMENTS Previous experience in IDD services, healthcare, or human resources. Familiarity with Oregon IDD systems (CDDPs, brokerages, DHS). Bilingual (Spanish preferred, but not required).
