Home Care Intake Coordinator

Interacts closely with facility representatives, discharge planners and other agencies to facilitate the initiation of care.Essential DutiesProvide explanation of the benefits to patients, families, and community contactsAssist with finding alternative services when patient is not a candidate for hospiceOversee scheduling of initial nursing assessment and admissionMaintain admission packets and ensure that proper forms are available to admissions staffKnowledgeable in all Home Health and/or Hospice processesAssist community outreach representatives with marketing activities to promote relations with referral sourcesAssist with completion of admission paperwork as neededAdhere to work schedule and maintain good punctuality and attendance recordProvide telephone and clerical support to other staff members as time allowsMaintains accurate clinical records, keeping track of goals and progress of clients’Abides by all regulations, policies, procedures and standardsPerform other duties as assignedPerformance ResponsibilitiesMaintains positive internal and external customer service relationshipsMaintains open lines of communicationPlans and organizes work effectively and ensures its completionMeets all productivity requirementsDemonstrates team behavior and promotes a team-oriented environmentActively participates in continuous Quality Improvement, educational programs, and staff meetingsRepresents the organization professionally at all timesPosition Requirements & CompetenciesBe eighteen (18) years of age or olderAt least one (1) year experience in a homecare, hospice and/or healthcare related field preferredAbility to complete necessary documentation appropriately, legibly and timelyAbility to handle stressful and delicate situationsInterpersonal, verbal, and written communication skills necessary to complete job responsibilitiesMust have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportationWORKING CONDITIONS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
