Experienced RN Hospice Case ManagerJoin our team as an experienced RN with hospice case management expertise. Manage a caseload of 15 patients, conducting 4-5 visits per day based on area and patient acuity. Complete charting on the same day. Perform all service level nursing, including routine, GIP, continuous, and respite care.
