From post-surgery and post-hospital rehabilitation to long-term care for a chronic illness, our highly skilled care teams provide compassionate attention and specialized care every step of the way.The Physical Therapist plans and provides physical therapy services to the clients in their home in accordance with the Plan of Care and the company's policies and procedures. The Physical Therapist performs functions which require substantial specialized knowledge, judgement and skill based upon the principles of psychological, biological, and social services and must be able to make judgements accordingly. The Physical Therapist reports directly to the Branch Manager.This is a per-diem positionDuties and Responsibilities:1. Administers physical therapy evaluations. Participates in the development of thePlan of Care.2. Provides physical therapy treatments, procedures, evaluations, and diagnostictests for clients under the direction of their physician for whom therapy has beenmedically prescribed.a. Translates all exercises into functional activities or activities of daily living.b. Provides instructions in the use and care of assistive devices.c. Develops needed plans for modifying equipment, appliances, and thephysical surroundings in client's homes.3. Prepares clinical and progress notes. Reports to physician and the BranchManager the client's response to treatment or change in condition.a. Maintains appropriate records including frequency of visits and client'sresponse.4. Instructs and supervises PTAs, HHAs, LPNs, and RNs in therapy program whenappropriate.a. Provides in-service education programs for company staff as requested.b. Participates in Case Conferences as indicated.5. Provides families with information, support, and encouragement to help motivateclients in their progress. Instructs client's family on home physical therapyprogram.a. Assists individuals and families to accept and adjust positively to physical,mental, and social limitations.b. Participates in the development of community resources to meet theneeds of clients.6. Maintains positive and effective communication with all staff and others.Understands and participates in team concept.7. Maintains absolute confidentiality of all information pertaining to clients, families,and employees.8. Maintains a safe client environment and identifies and reports to the office anysuspected vulnerable client abuse, neglect, or financial exploitation.9. Performs other related duties and responsibilities as assigned by the BranchPhysical/Environmental Demands:Heavy to Very Heavy. Involves lifting clients, bending, stooping, stretching, and settingup equipment. Must be adaptable to a variety of environments and community settings.Must be able to drive or use public transportation in all types of weather.Exerting more than one hundred pounds of force occasionally, more than fifty pounds offorce frequently, and more than twenty pounds of force constantly to move objects.Physical demand requirements are more than those for heavy work.Qualifications:1. Graduate of an accredited Physical Therapy program.2. Currently licensed as a Physical Therapist in the state of Massachusetts. Thelicense must not have been revoked, suspended, and without limitations orrestrictions.3. Have at least two (2) years experience in a health care setting or equivalentexperience. Home care experience preferred.4. Strong interpersonal communication and teaching skills.5. Strong organizational skills and the ability to work independently.6. Excellent written and oral communication skills.7. Access to a dependable vehicle or public transportation in order to travel tomultiple company business stops a day.8. A valid driver's license and proof of car insurance when using a personal vehiclefor company business.9. Proof of negative PPD or documentation of negative chest x-ray.10.Current CPR certification is highly recommended.11.Pass initial and ongoing background studies and screenings including but notlimited to those of the Massachusetts Department of Health and the FederalOffice of the Inspector General's List of Excluded Individuals and Entities.
