Job Details: Position:- RN Case Manager Location:- 18300 US Highway 18 Apple Valley, CA 92307 Position Type:- Contractual Duration:- 13 weeks (Possible Extension) Shift:- Day shift: (5x8 hours) 8:00am - 4:30pm Payrate:- Travel Pay:- $2900 -$3000/week on W2 (Without Benefits) Local Pay:- $54/Hr on W2 (Without Benefits) Required: Current California RN license. BLS from AHA. Two years clinical experience. 2 years of Case Management experience and or certification in Case Management. Job Summary The Registered Nurse Case Manager plays a critical role in coordinating patient care across various settings, ensuring that patients receive comprehensive and effective healthcare services. This position involves working closely with patients, families, and healthcare teams to facilitate optimal health outcomes through effective case management practices. The ideal candidate will possess a strong clinical background and demonstrate exceptional communication and organizational skills. Duties Conduct thorough patient assessments to determine individual healthcare needs and develop tailored care plans. Collaborate with interdisciplinary teams to coordinate patient services, including acute care, outpatient services, and post-acute care. Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes. Provide education and support to patients and families regarding treatment options, discharge planning, and resources available. Maintain accurate medical records in compliance with HIPAA regulations and ensure proper documentation of all patient interactions. Utilize critical thinking skills to evaluate diagnostic information and implement appropriate interventions. Engage in utilization management activities to optimize resource use while ensuring quality patient care. Advocate for patients by navigating complex healthcare systems and addressing barriers to care. Qualifications Valid Registered Nurse (RN) license in the state of practice. Critical care experience is preferred; experience in a Level I or Level II trauma center is highly desirable. Background in ICU, emergency medicine, hospital medicine, or medical-surgical nursing is beneficial. Experience with pediatrics, geriatrics, behavioral health, infectious disease care, or outpatient clinic settings is a plus. Strong knowledge of medical terminology, physiology, and case management principles. Proficiency in patient assessment techniques and vital sign monitoring. Familiarity with discharge planning processes and the management of medical records.
