Home Health Physical Therapy

Overview We are seeking a Physical Therapist per diem to join our team! As a Physical Therapist, you will be meeting with patients at their home, discussing their treatment needs and ultimate goals, and crafting customized, comprehensive therapy plans for them. You will also work closely with other members of their health team to arrange follow-up care, help educate patients on lifestyle changes they may need, and provide a positive, affirming presence in their lives.
