PRN Physical Therapist

PRN Physical Therapist will provide direct patient care to home bound patients. Pay will be per visit. Responsibilities and Duties: • Develops and revises plan of care in consultation with physician and other care-team members. • Provides therapeutic treatments and evaluation of equipment needs as applicable. • Participates in clinical conferences and in-service programs. • Conducts on-site supervisory visits of physical therapy assistants. • Advises and consults with the family and other agency personnel as applicable. • Prepares clinical and progress notes. • Protects and honors patient and coworker confidentiality and right to privacy. • Evaluates outcomes of care. • Plans for discharge. Qualifications & Skills: • - Current Texas PT License. • - Current Certification in CPR • - Valid Texas Driver's License and Automobile Insurance • - Knowledge of WellSky EMR software is a plus Salary: Pay Per Visit including mileage reimbursement and points assigned on drive time for additional pay.
