This hybrid role works both virtually and in-person within a collaborating OB/GYN office, partnering closely with providers, clinical staff, and patients to deliver coordinated, patient-centered care across pregnancy and the postpartum period.If you are passionate about maternal health, health equity, and supporting families during one of life’s most important transitions, this role offers the opportunity to make meaningful impact every day.Why This Role MattersThe pregnancy and postpartum periods are critical windows for long-term maternal and infant health. As a Patient Care Coordinator, you will play a vital role in improving access, reducing barriers, and strengthening continuity of care for families during this transformative time.ResponsibilitiesPatient Engagement & Relationship-Based SupportEngage pregnant and postpartum patients across risk levelsConduct outreach via phone, text, video, and in-person visitsExplain care management services and support program enrollmentBuild trust using trauma-informed and culturally responsive communicationCare CoordinationAssist patients in understanding and navigating health plan benefitsCoordinate appointments with OB/GYNs, PCPs, pediatricians, specialists, and behavioral health providersAddress social determinants of health including transportation, housing instability, food insecurity, and childcareCollaborate closely with OB/GYN providers and interdisciplinary teams to ensure seamless transitions of carePatient Education & EmpowermentProvide education using approved materialsPromote self-management and patient empowermentReinforce postpartum follow-up, well-child visits, and preventive care adherenceDocumentation & TechnologyDocument all encounters and care plans accurately in the EMRParticipate in virtual case reviews and team meetings via Microsoft Outlook and TeamsMaintain patient registries and support reporting initiativesComplete administrative duties as assignedQuality & Program SupportContribute to quality improvement initiatives focused on maternal health outcomesSupport performance metrics such as postpartum visit completion and preventive care adherenceAdhere to organizational policies, care pathways, and safety standardsQualificationsHigh school diploma or equivalent required with minimum of 2 to 4 years of relevant healthcare experience in care coordination, OB/GYN practice support, or patient navigation in a hospital or medical office.Associate or bachelor’s degree in a health care related field preferred. LPNs with relevant experience will be considered.2+ years of experience working with perinatal populations and/or NICU experience preferred.Strong communication and interpersonal skillsExcellent organizational and prioritization skillsExperience working with diverse and vulnerable populationsDemonstrated ability to work with a high degree of autonomy in both virtual and clinic settings.Comfort working independently and as part of a multidisciplinary teamKnowledge of Medicaid and commercial payor plans a plusWe’re here for you—both personally and professionally.Our benefits are designed to support you and your family at every stage of life.
