The organization is committed to providing high-quality, compassionate primary care and improving access to healthcare for underserved communities through an integrated, patient-centered approach.Position SummaryThe Nurse Practitioner or Physician Assistant provides comprehensive, patient-centered outpatient primary care to individuals across the lifespan. This role focuses on delivering high-quality care in a collaborative, team-based environment while addressing both medical and social determinants of health.
