The Outpatient Clinician - Telehealth is part of a Virtual Hub team that provides clinical services for individuals and families that present for telehealth treatment. The Virtual Hub is part of an integrated model of care and is complemented by physical office locations with services that can be offered in-person, in the community and/or via telehealth to provide a holistic healthcare experience. In collaboration with a treatment team, the Outpatient Clinician - Telehealth provides services including but not limited to treatment planning and on-going assessment; client education; crisis intervention and risk assessment; individual, family, group and collateral therapy, referral and advocacy. Caseloads include diverse populations with a variety of clinical needs. The Outpatient Clinician - Telehealth is a positive team player that promotes a culture of inclusion and participation.Required Education and ExperienceMaster's degree in Marriage and Family Therapy, Professional Counseling, Psychology, or Social WorkLicensed Independent Practitioner by Kentucky professional boards and billing sources (e.g. LCSW, LPCC, LPP, LMFT, PhD clinical psychologist, etc.)Meet the requirements of applicable Kentucky licensure boards to provide services to individuals and families residing in the state of KentuckyPreferred Education and ExperienceExperience working in a behavioral health fieldAdditional Eligibility QualificationsEssential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Clinical ServicesWork as part of a team to deliver high quality therapeutic care from a whole health perspectiveDeliver evidenced based counseling that provides effective medically necessary treatment of mental and behavioral conditions in individuals and familiesAssess and integrate historical, psychosocial and medical information into a client's individualized treatmentMonitor the need for continuation of services and/or change in level of intensity of servicesUtilize PCOMS (Partners for Change Outcome Management System) in assessments and clinical treatment episodesDeliver services primarily in outpatient home-office settingDeliver services via telehealth and coordinate with the treatment team to ensure client needs are met in person or via telehealthManage situations involving client risk and provide clinical crisis services to de-escalate crisis situations and promote stabilityManage a caseload that may include diverse populations with a variety of clinical needsMaintain a professional and secure home-office environmentEngage treatment team, New Vista specialty programs and community partners to advance relationships to provide whole health care to address behavioral health, physical health and social determinant needsCollaboration and CommunicationCollaborate with client, family (if appropriate) and treatment team to develop treatment goalsCommunicate effectively with team members and collateral sourcesRefer and transfer when clinically indicatedParticipate in regular meetings with Clinic Manager and treatment teamMaintain an integrated care approach by engaging client's local resources including care team (internal and external to New Vista), family, friends, school, and other community resources, as appropriateDocumentationManage medical records for clients receiving servicesSecure authorizations for medically necessary clinical servicesDocument to meet medical necessity standardsSubmit documentation within required timeframesWhile we welcome all to apply, there are some position specific background stipulations that must be met to comply with various regulatory and contractual requirements.
