Home Healthcare Social Worker

Job Summary This Social Worker position will be the primary person responsible for in-home assessments for cleaners, as well as providing support for care giver and social companion assessments, meeting with the clients to assess what their needs are and if the home is safe. They will also conduct the county required reassessments every three months. The information gathered in these assessments is the basis of the client’s care plan, the document that guides all further interaction and connections with the client. Additionally, this Social Worker will be the formal supervisor and mentor for the social work interns at Surrey. Primary Job Responsibilities Conduct comprehensive in-home assessments as to the suitability of assigning Surrey Home Care employees to clients Develop and implement care plans in collaboration with the interdisciplinary home health team Advocate for clients’ rights and needs with outside service providers and community organizations as needed and appropriate Participate in interdisciplinary team meetings and case reviews to evaluate care plans and outcomes Maintain accurate, timely and confidential documentation in accordance with agency policies and regulatory standards Serve as advisor and field instructor for social work student interns Provide supervision, training, and evaluation for the social work interns in accordance with university and agency requirements * Qualifications Master’s Degree in Social Work (MSW) from an accredited program required Current state license as a Licensed Social Worker preferred Minimum of 2 years of social work experience in home health, community health or geriatric care preferred Strong assessment, case management and advocacy skills Excellent interpersonal and communication abilities, both verbal and written Ability to work independently and collaboratively within a multidisciplinary team Knowledge of community resources, social service systems and home care regulations Experience supervising students or interns preferred Patient demeanor with the ability to relate to, empathize with, and show compassion for a variety of personalities Pay: $50,000.00 - $55,000.00 per year Benefits: 401(k) 401(k) matching Dental insurance Employee assistance program Flexible spending account Health insurance Life insurance Mileage reimbursement Paid time off Work Location: In person
