Team Lead, Care Management Mom & Baby
Job Summary:The Team Lead, Care Management provides front line management and support to departmental leadership in the development and execution of the Care Model in alignment with corporate goalsEssential Functions:Functions as a "hands-on" supervisor, assisting in day-to-day operations and supervision of staff with adherence to departmental expectationsResponsible for recruiting, hiring, onboarding, training and ongoing support of the departmental teamConducts virtual meetings as needed to train, lead, and support staff towards the effective administration of CM programPrepares and utilizes reports to monitor and audit all systems, procedures and the quality of customer services provided. Assist with representing the unit at Departmental and Planning meetings as requestedEnsures staffing policies, procedures and SOPs are standardized and are current to meet business needsAssists the Managers as needed in the review and development of new projects/processes as requested by the health plansOversee appropriate system documentation within defined protocols and guidelinesIn support of the Strategic Plan, serve as the technical and content support to align programs/processesServe as interdepartmental liaison, collaborating with internal resources in monitoring service levels to meet requirements and analyze for trendsParticipate in meetings with internal/external resources to provide information about services and benefits available to members and referral mechanismsAssist in arranging and coordinating services with community partners to maximize utilization of appropriate community resources and support groupsParticipant in development of budget and perform ongoing monitoring of statusAssist with implementation of interventions to maintain and/or improve the quality and efficiency of the operations/services providedPerform any other job duties as requestedEducation and Experience:Completion of an accredited registered nursing (RN) degree program, Bachelor's Degree in Social Work, or Master's degree as relevant to licensure eligibility is requiredA minimum of three (3) years of healthcare experience is requiredA minimum of two (2) years of managed care and/or case management experience is preferredA minimum of three (3) years experience in maternity is preferredPrevious supervisory experience is preferredCompetencies, Knowledge and Skills:Ability to display a customer service, member-focused orientationStrong data analysis and trending skillsStrong team and staff development skillsStrong collaboration and conflict resolution skill setsAbility to develop, prioritize and accomplish goals/time managementStrong decision making and problem solving skillsExceptional communication skills (both written and verbal) with the ability to present information in a variety of different formatsEffective listening and critical thinking skillsStrong ability to teach best practice, engagement strategies for successful member participation (including motivational interviewing, best practice engagement techniques)Knowledge of state and federal healthcare regulations and environmentAbility to work in a fast-past environmentAbility to work independently and within a team environmentAdvanced knowledge of Microsoft Word, Excel, PowerPoint and VisioAbility to operate smart phone, iPad or other mobile communication devicesLicensure and Certification:Active, unrestricted Registered Nurse (RN) license, Social Work license, or Counselor license is requiredCase Management certification is preferredWorking Conditions:General office environment; may be required to sit or stand for extended periods of timeCompensation Range:$72,200.00 - $115,500.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
