Child Placement Team LeadAt Easterseals PORT Health, our mission is rooted in empowering individuals and strengthening communities.We are seeking a skilled and mission-driven Child Placement Team Lead to coordinate and supervise therapeutic foster care placement services for children and families. In this leadership role, you will oversee placement operations, guide staff, ensure regulatory compliance, and support high-quality, person-centered services that promote stability, safety, and positive outcomes for children in care.This isn't just a leadership role it's an opportunity to influence systems of care, support frontline staff, and help children and families thrive.As the Child Placement Team Lead, you will provide day-to-day leadership and oversight of child placement services within Child Placement and Support Services. You will work closely with the Program Supervisor, Child Placement Coordinators, foster parents, and contracted agencies to ensure that placements are appropriate, services are authorized and delivered without gaps, and all care meets or exceeds state, local, Medicaid, and ESPH standards.You will play a critical role in screening referrals, coordinating placements and discharges, supervising staff, responding to crises, and ensuring adherence to quality improvement, client rights, and confidentiality standards.Leadership & SupervisionProvide daily guidance, coaching, and on-site supervision to Child Placement Coordinators.Observe and provide feedback on Foster Parent Clinical Supervision sessions.Support staff performance, training completion, attendance, and adherence to ESPH policies.Assist the Program Supervisor with referral screening and placement decision-making.Placement Coordination & OversightCoordinate and facilitate pre-placement meetings for all accepted referrals.Ensure all intake documentation is complete and records are established prior to placement.Maintain a small caseload (up to 5 children) to model best practices.Coordinate discharge planning with contracted agencies, including completion of discharge summaries within 24 hours and full chart closure.Service Authorization & ComplianceSubmit initial authorization requests prior to placement.Monitor and manage all authorizations to ensure uninterrupted services.Review documentation to ensure service plans, goals, and progress notes meet Medicaid and regulatory standards.Ensure accurate and timely preparation and submission of billing documentation.Quality, Communication & Crisis ResponseParticipate in Quality Improvement activities and support continuous service improvement.Conduct home visits to monitor service delivery and compliance.Communicate regularly with contracted agencies regarding child and family status, incidents, medication changes, educational performance, and placement changes (within 24 hours).Respond to emergency and crisis situations, including evenings, nights, weekends, and holidays as needed.Uphold Client Rights, confidentiality standards, and HIPAA compliance at all times.Program Growth & RepresentationAssist in the expansion of child residential and placement services through presentations to community and contracted agencies.Represent Easterseals PORT Health professionally with families, foster parents, and partner organizations.Attend required trainings, seminars, and professional development activities.Why Join Us?At Easterseals PORT Health, leadership means partnership.
