Ready to practice medicine the way it was meant to be—unhurried, compassionate, and patient-centered?Senior Doc is expanding our Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for seniors and patients with serious or chronic illness.This role offers the ideal balance of autonomy and collaboration, allowing you to make a genuine impact while maintaining work–life balance across multiple care settings.Why Senior Doc?At Senior Doc, we bring high-quality medical care directly to patients, restoring dignity, comfort, and trust in healthcare across inpatient, community, and home-based environments.What you’ll enjoy: Flexible Scheduling: Consistent schedule designed to support work–life balance Stability & Growth: Long-term career pathways within a growing, mission-driven organization Supportive Culture: Collaborative, interdisciplinary team focused on patient success and provider satisfaction Position Overview: We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions.This position supports inpatient palliative care, community-based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.Key Responsibilities: Provide comprehensive palliative care consultations across inpatient, community, and home-based settings Develop and manage individualized care plans emphasizing comfort, symptom control, and patient goals Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams Serve as a clinical leader within the interdisciplinary team Educate patients and families on disease progression, treatment options, and goals of careDemonstrate strong time management while managing a diverse patient caseload Participate in interdisciplinary meetings, case conferences, and care planning Travel within the community to conduct home visits as neededQualifications: Active California Nurse Practitioner or Physician Assistant license Palliative Advanced Practice Clinician experience required Experience in inpatient palliative care and/or community-based palliative care Strong interdisciplinary team leadership skills Excellent time management and organizational abilities Comfortable with home visits and community driving Minimum 1–3+ years of palliative care, hospice, or serious illness care experienceStrong communication skills with patients, families, and care teams Ability to work independently while coordinating care across settingsPreferred / Plus Qualifications: Advanced Certified Hospice and Palliative Nurse (ACHPN)Credentialed at either Orange, LA, Riverside, San Diego County area hospitalsPrior experience working across multiple care environmentWhat We Offer: Flexible, provider-centered work environment Opportunities for professional growth and leadership development Supportive, mission-driven, interdisciplinary cultureMust have driver’s license and reliable source of transportation and insurance.
