Job Description Description:We are Hiring!Job Posting: LICENSED PRACTICAL NURSE - FLOATINGLocation: Rochester, NYDepartment: NursingEmployment Type: Full-TimeSchedule: Monday thru Friday, 7am to 3:30pmSalary: $25 to $30/hrGeneral DescriptionFloat LPN provides flexible staffing across various program settings, ensuring high-quality patient care and adapting to diverse clinical environments. Under Nursing supervision, provides medication management and offers general health and wellness support. Works collaboratively with other providers in the residence to facilitate client wellness. Maintains confidentiality and ethical standards of relevant governing bodies. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.Essential Duties and ResponsibilitiesProvides medication management within the residential settingProvides oversight during residential self-medication process to clients in the residential environment in accordance with authorized prescriptionsDocuments the medication self-administration process in each client's electronic Medication Administration Record (EMAR)Procures and logs in medications received for each client within the facilityTracks client compliance with required medications, identifying any concerns to other Prescribers and other medical staffReports medication compliance to the clinical teamCommunicates verbally with NSM and residential staff when calling off for unplanned absences.Provides Day to Day support of client wellness within the settingObserves and identifies potential client medical or medication concernsFacilities identification of medical concerns to appropriate medical or psychiatric staff or prescribersAssures identified medical concerns are addressedCommunicates and collaborates with appropriate Agency staff, medical care providers, and related community organizations to assure coordination of client care and services.Completes required paperwork related to communication or services providedCoordinates with appropriate residential staff to facilitate transportation to necessary external medical appointmentsPromotes health and wellness within the residential facilitySupports the development of client wellness education and activitiesAssists Medical Staff and PrescribersProvides Support with vital signs, and other assistance to providers as neededCollaborates with appropriate residential staff on client discharge plansParticipates in staff meetings and medical/clinical supervision as directedParticipates in relevant Agency meetings and/or trainingsActively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Employees are required to follow appropriate supervisory direction and perform other related duties as required.Requirements:Qualifications:Education: Minimum of a High School Diploma or GED and accredited LPN diploma required.Credentials: Valid NYS Licensed Practical Nurse requiredExperience: Two to three years of experience related to nursing services to individuals with mental health, and chemical dependency / substance use disorder related services preferred.
