Job Responsibilities Assess patient strengths, limitations, and needs using standardized measurement, prediction, and evaluation testsDevelop and implement individualized short‐ and long‐term treatment goals based on patient assessmentsAdminister and interpret clinical physical therapy services, adjusting treatment plans as needed based on patient responseMaintain accurate and timely patient treatment records, documenting progress and changes in electronic health systemsEnsure all documentation meets requirements of reimbursement and regulatory agencies, completed within two business daysEstablish and maintain effective working relationships with patients, families, and interdisciplinary team membersParticipate in patient care conferences and collaborate with other healthcare professionals for coordinated careProvide education to patients and families regarding treatment plans, safety, and self‐care techniquesParticipate in discharge planning and make appropriate referrals, coordinating with vendors for durable medical equipmentSupervise and provide direction to Physical Therapist Assistants during treatment sessionsMaintain licensure requirements and pursue continuing education to stay current with professional standardsEngage in departmental and organizational performance improvement initiatives to enhance quality of carePosition Qualifications Minimum Required:Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New YorkGraduate of an accredited school of physical therapy with a Bachelor's or Master's Degree or equivalent in physical therapyCurrent CPR certificationPrevious experience in a licensed healthcare settingPreferred:Master's or Doctorate Degree in Physical TherapyPrevious physical therapy experience in a hospital‐based settingWork EnvironmentMajority of work is performed in the patient home. Work in the office requires working in close quarters with other employees. The offices are air conditioned/heated.
