Job DescriptionWhat is a Clinical Transformation Advisor:The Clinical Transformation Advisory team utilizes the Lightbeam platform to optimize care management processes, improve clinical outcomes, and disseminate best practices to our client partners. This position is a client facing role and has direct reports. The CTA’s is primarily responsible for successfully implementing care management processes that are operationally sustainable, promote high-value care, and prepare organizations to succeed within their value-based contracts and other payer initiatives.The ideal teammate will have successfully demonstrated clinical consulting experience with the ability to come up to speed on the population health industry and Lightbeam platform quickly. Expectations for this role include building and managing successful relationships with both new and existing clients, identifying their needs and goals to help them succeed using our population health management tools, and collaborating closely with internal stakeholders to help drive client and company success.
