Position Overview This hybrid position offers the flexibility of working both in the clinic and from home, allowing you to balance patient interaction with administrative tasks remotely. Key Responsibilities Administrative Duties: Schedule and manage patient appointments efficiently. Maintain and update electronic health records (EHR) with precision. Handle billing and insurance coding accurately. Clinical Duties: Explain Chronic Care Management (CCM) effectively: Educate patients on the benefits and goals of CCM, which aims to improve quality of life by managing chronic conditions such as diabetes, hypertension, and heart disease. Coordinate care by collaborating with pharmacists, dieticians, and social services to create personalized care plans. Monitor patients' progress and adherence to care plans, providing support and resources as needed. Facilitate communication between patients and healthcare providers to ensure continuity of care. Qualifications Required: Certified Medical Assistant (CMA) certification. Experience with EHR systems. Knowledge of medical terminology and physiology.
