LPN - Full Time
Shift: 3pm-Midnight, 4 days per week. Includes alternating weekends and holidays.

The Licensed Practical Nurse (LPN) is responsible to supervise caregivers and med techs and oversee the day to day care of the residents. The LPN acts as the facility supervisor in the absence of the administrator or designee.
Qualifications and Requirements.
The LPN applicant must:
1. Be a graduate of an approved school of nursing with a current license to practice in the state of Pennsylvania or eligible for licensure in PA;
2. Provide a valid high school diploma and driver’s license;
3. Have at least one year’s experience in the health care field, preferably with older adults;
4. Be at least 21 years of age;
5. Provide record of current CPR certification;
6. Be in good physical health and able to lift and carry a minimum of 25 pounds; and
7. Pass a criminal background check.
Responsibilities.
1. Demonstrate good personal hygiene in dress and appearance;
2. Be self-motivated and detail oriented in day to day responsibilities;
3. Communicate effectively and act professionally in all interactions with residents, family members, and visitors;
4. Be respectful and courteous to residents and families and place priority on their individual needs and requests;
5. Partner with co-workers to provide highest standard of customer service to residents;
6. Be familiar with the Department of Human Services regulatory guidelines for personal care homes;
7. Respond to and follow-up appropriately to resident and/or family grievances;
8. Assess residents using a holistic approach and respond appropriately to any physical, emotional, psychological, social and/or spiritual needs;
9. Collaborate with physicians, their office employees and other health care settings and systems involved in each resident’s plan of care to ensure best resident outcomes;
10. Consult with residents and/or family members to develop and update the resident assessment and support plan;
11. Assist residents with making appointments and transportation arrangements;
12. Maintain the lab log and complete lab slips to ensure resident bloodwork is drawn according to physician orders;
13. Complete wound care and treatments as ordered and report assessment findings appropriately;
14. Coordinate resident admission, discharge and transfer processes;
15. Collaborate with the resident, physician(s), pharmacist and family members to review and reconcile the resident’s active medication list on admission and thereafter;
16. Assess residents’ ability to self-medicate and assist them as appropriate to maintain independence with this task;
17. Administer medications to residents as prescribed and oversee the efficient operation of the Med Room and the facility in general;
18. Maintain accurate resident records;
19. Protect the privacy and confidentiality of resident information;
20. Be present in the dining room to supervise mealtimes and assist as needed;
21. Assist residents with personal hygiene/care needs as requested;
22. Promote a safe work environment and respond appropriately to safety concerns;
23. Teach employees proper care techniques, about new processes/procedures, the standards of medication administration, to competently use equipment, about disease processes affecting residents, and whenever appropriate;
24. Supervise caregivers and med techs and mediate problems/concerns appropriately;
25. Evaluate employee job performance(s) as requested by the administrator or designee;
26. Provide employee disciplinary action as appropriate;
27. Report to work for shifts as scheduled and be flexible and willing to work different days and various shifts as needed to best meet resident needs;
28. Lead fire drills and instruct employees in the execution of the fire and disaster plan;
29. Attend mandatory in-services, trainings, and meetings as well as continuing education opportunities as needed to meet licensure requirements and for professional growth;
30. Identify opportunities for performance improvement and participate in performance improvement activities as needed;
31. Adhere to facility policies and procedures.
