Contractual RN-Public Health Nurse II (PHN II)

The Tuscola County Health Department is seeking a Registered Nurse (Public Health Nurse II) to work on a contractual basis. This position will work up to 3 days per week not to exceed 999 hours in a fiscal year. This position works in various programs under the supervision of the Nursing Administrator and provides skilled nursing services to individuals, families, and groups in their home, clinic, school, work, and/or community environment. More information can be found at www.tchd.us Job duties may include but not limited to: Provides home visits (not home health) to families or individuals across the life span including individuals with special health care needs. Promotes healthy pregnancies, positive birth outcomes, and healthy infant growth and development. Develops care plans, conducts needs assessments, and provides case management and care coordination services. Assists individuals with special health care needs in accessing the broadest possible range of appropriate medical care, health education, and supports. Educates the client, family, and community regarding care, prevention strategies, etc. and assists the client in decision making/goal setting. Administers immunizations and medications according to physician’s orders and Health Department policy. Performs epidemiological investigations and follow-up. Prepares reports, composes letters, creates education and outreach materials. Performs Emergency Response roles as required. Requirements: MUST possess a current Michigan licensure as a registered nurse by the Michigan Department of Licensing and Regulatory Affairs; and at least one year of experience providing community health, pediatric or maternal/infant nursing services preferred. MUST possess a valid MI driver’s license without limitations. A drug test will be required.
