Job Title: Registered Nurse (RN) – Admission Assessment (PRN / Per Diem)Employment Type: PRN (As Needed)Compensation: $30–$50 per completed assessmentPosition SummaryThe Registered Nurse (RN) – Admission Assessment is responsible for conducting comprehensive pre-service, in-home clinical evaluations for prospective clients prior to the initiation of care.This PRN position plays a critical role in establishing safe, appropriate, and individualized care plans based on a thorough assessment of the client's medical condition, functional status, and home environment. The RN ensures compliance with regulatory standards, agency policies, and evidence-based clinical practice.Essential Duties and ResponsibilitiesPerform comprehensive in-home admission assessments for new clientsObtain and review medical history, current diagnoses, medication profiles, and treatment plansConduct physical, functional, cognitive, and psychosocial evaluationsAssess home safety and environmental risk factorsIdentify skilled and non-skilled service needsDevelop and document individualized care plans in accordance with physician orders and agency protocolsCollaborate with interdisciplinary team members to ensure appropriate service coordinationEnsure timely and accurate completion of documentation within established compliance standardsMaintain adherence to federal, state, and local regulations governing home health servicesProtect patient confidentiality in accordance with HIPAA guidelinesMinimum QualificationsActive, Registered Nurse (RN) license in the state of North CarolinaMinimum two (2) years of clinical nursing experienceDemonstrated proficiency in patient assessment and care planningStrong clinical judgment and critical thinking skillsExcellent written and verbal communication abilitiesReliable transportation and ability to conduct home visitsCurrent CPR certificationPreferred Qualifications:Prior home health, hospice, or community-based nursing experienceKnowledge of state home care regulatory requirements
